I went to my grandparents' last night for an early Christmas party. Both of my mom's parents live in Pawtucket along Central Ave on opposite sides of newport Ave. My mom's mom and step-dad live in the same side of Newport Ave as the old WalGreens and my mom's dad ans step-mom live on the same side of Newport ave as Slater Park. Niether live on Newport ave though. My mom's mom's street has 100W MV M-250R and M-250A street lights and my mom's dad's street has 70W HPS M-250R2s on it.
You got some of them right... The only M-250R2 is the one in the foreground. It was installed in 2010 and replaced a FCO M-250R2. Yep about the OVH. Nope that's not a M-400A (are you on your phone? ) but yeah two GE PF-400s, one's the older version and the other is the newer version. Yea, the signals are eagle Mark IV aluminum signals. There are a couple more lights.
Do you want to guess wattages? I'll give you some hints:
The small wattage lights are all the same wattage except the M-250R2, which is the same wattage as the floodlights and M-400A2. Everything is HPS as well.

EDIT: Hmm the other M-250R2 is actually a M-250R1, and the M-400A is actually a M-400 split door? Anyways I guess the M-400 split door, floods and M-250R2 is 250w HPS, while the M-250R1 and OVH is 70w HPS.
