Philips F40WW, my first one!
I was glad to get my hand on one of those! Not NOS but still has moderately low hours. Plus it's been made at Trois-Rivières, QC! It had the modern Philips caps and by 1986 the bold Philips logo was dropped, but still had the big shield logo.

i like the etch on that lamp! nice find vince !

No electrode guards? The etch looks very similar to those lamps made at the Salina, KS plant.
