Philips 40w Lumiline II 
Recent Restore find. 

Philips Lumiline II lamps are basically Lumiline lamps but with the endcaps for Leviton style sockets permanently attached. I'm guessing they did this to simplify installation but the disadvantage is that they don't fit fixtures with GE style sockets. Not sure when Philips stopped making these, this one is from 1991 or older as that's when Pascal Hardware closed. 

I believe only NOS lumiline lamps (at least incandescent ones) are available at this point, with [url=http://genet.gelighting.com/LightProducts/Dispatcher?REQUEST=COMMERCIALSPECPAGE&PRODUCTCODE=15839&BreadCrumbValues=CATG,_Lamps_Incandescent_Tubular_T8&SearchFieldCode=null]GE[/url] the last of the big three to make them. I think they made them up until 2009 or so which was the last time I saw them on store shelves. Surprisingly replacement endcaps are still made along with drop in LED retrofit tubes.

Yeah or maybe shortly before as that's the last time I saw them store shelves. They had sleeves like this and I think by then they already transferred production to Mexico.
