Artcraft F15 Tandem Striplight
Here's a tandem version of the slim F15 striplights that I found at Restore, this one is pretty old and was made by Artcraft. 

It uses some odd slimline choke ballasts that look pretty old, there's a typo on the ballast label. 

The lamps that it came with are F15T8 daylight Sylvanias, the starters are A.H. & H. and Artcraft branded both Spec. No. 6 (gotta watch out for that wax cap O_o). I guess if I were to use it I would get some modern FS-2s for it though.

I have several daylight Sylvania F15T8/Ds like those! I think they are GE-made.

What's wrong with wax caps? How do you know if a starter has them?

Well, they tend to dry out over the years and short out/fry mmm crispy. They're usually found in older starters that say "Spec No. 6" on them but to make sure I usually take them apart to check if the starter looks old enough. To make them safe you can cut out the wax cap or replace it with a new ceramic or mylar one.
