This is an American Electric Ballast in Arm!
The Manufacturer is American Electric
The Model is Ballast in Arm...
I used to have one, a simple bucket assembly replacement will solve that problem!
A new note...this is not a BT lamp...it's a ET 23.5 shaped lamp and this is a Sylvania 150 watt HPS lamp!
This ET 23.5 shape pretty much replaced the old classic BT-25 shape.
