Has the newer pinkish colored phosphors... I'd guess this is a late 80s to early 90s lamp? i'll post the date code after i look at it again...

It is early-mid 80s, from the package style. The date code should be on the base or on a shiny metal tab inside the bulb around the neck area. It would start with SR then three numbers.

This lamp's code is SR112.

Mfd 4/90, thats late for that sleeve style. Sleeves at that time generally had a white corner with many dots surrounding it.

Joe told me this is indeed the original sleeve.
