According to the model number, This is a CWA unit, FCO, with an open plug-in ignitor

And was wired for 120V but it has a multi-tap ballast.

Yeah, my 2005 125's tag says 240 volt but it was also a multi-tap ballast. It was changed to 120 by the time I got it anyways.

Yeah it was FCO. I have seen this light in service a lot. I drove under it many times on C-470 and platte canyon road, where it was lighting the road. Last and loner 125 on a curved upsweep in a chain of straight arms with AE Luxmasters on them. Plus the Relflector has grey paint.
