If you look at the ballast label, it says that it is a "M175/SUPER5-KIT" And it lists on the ballast that it takes a 175 watt MH and MV lamp. (M57 and H39) Even though the ballast is what was in the High-Bay, it is a Sylvania ballast and it says it's suitable for both. The clear BT mercury vapor lamp I got from the Re-Store and it came with the new NEMA head I just got the other day.

The lamp is a Westinghouse Lifeguard lamp, might be kind of old. O.o # is H39KB-175. Dunno what the date would be, but I would have to look at it more to find the date.
